Patient Navigator

Discovering that you are about to battle cancer can be frightening, confusing and overwhelming.

At the core of our program is the Patient Navigator, whose services are provided free of charge. The role of the Patient Navigator is to assist our patients and families through the entire continuum of care.

This includes patients going through diagnostic work-up, newly diagnosed, already undergoing treatment, or patients needing follow-up.

Patient Navigator Eva Kourlas, RN, works closely with each patient to explain more than just the medical aspects of dealing with cancer. Eva, a Certified Breast Health Educator, strives to provide holistic care for the mind, body and spirit of each patient.

Goals of the Patient Navigator

  • Provide information and resources about diagnosis and treatment
  • Assist in coordinating physician and diagnostic appointments
  • Inform and educate about diagnostic tests, treatments, side effects, roles of physicians, and other aspects of care
  • Assist with questions to ask providers
  • Give referrals to support groups and other programs
  • Assess needs for financial assistance, home health care, insurance issues, medication assistance, transportation and other concerns
  • Serve as a point of contact for patients and families
